Our staff is available to support you in providing quality health services to your patients. We can assist you by providing a variety of services which will supplement the medical care you provide:

In-home assessment to evaluate the home environment and identify issues which may impact on your patient’s safety, compliance with recommended regimes, and well-being.
Consultation with your patient and/or their family to assist them in evaluating the need for, and accessing non-medical services.
Ongoing follow-up as needed.
